How is Medicare Part B Premium Calculated

You are required to pay a premium each month for Medicare Part B. In 2022, the Standard Part B Premium amount is 170.10.  If the Modified Adjusted Gross Income as reported on the IRS tax return from 2 years ago is above a certain amount, you would pay an additional Income Related Monthly Adjustment Amount (IRMAA) in addition to the standard monthly premium. IRMAA is an extra charge added to your premium.

Part B Premiums are automatically deducted from the benefit payment if you get benefits from either the Social Security Administration, Railroad Retirement Board or the Office of Personnel Management. If you don't get these benefits, you will receive a monthly bill. 

The premium you pay for Medicare Part B depends upon your yearly Income adjusted with marital status. For 2022, following are the premium amounts (based upon 2020 Tax Return):


Monthly Premium For a "Single" filer status based upon Income:

$91000.00 or Less, Premium Amount is $170.10

Above $91000.00 up to 114000.00, Premium amount is $238.10

Above $114000.00 up to $142000.00, Premium amount is $340.020

Above $142000.00 up to $170,000.00, Premium amount is $442.30

Above $170,000 and less than $500,000.00. Premium amount is $544.30

$500,000 or more, Premium amount is $578.30


Monthly Premium for a Joint Tax Return based upon Income:

$182000.00 or less, Premium Amount is $170.10

Above $182000.00 up to $228000.00, Premium amount is $238.10

Above $228000.00, up to $284000.00, Premium amount is $340.20

Above $284000.00, up to $340,000.00, Premium amount is $442.30

Above $340,000, up to $750,000.00, Premium amount is $544.30

$750,000 or more, Premium amount is $578.30


Monthly Premium for Married and filing a separate Return:

$91000.00 or less, Premium amount is $170.10

Above $91000.00 and less than $409,000.00, Premium amount is $544.30

$409000.00 or above, Premium amount is $578.30
